Parents: Please provide updates throughout the year when cell, home or work numbers change.
- Parents are asked each year to complete a new information form for use by the school staff in the event a parent or adult guardian has to be reached quickly for a student health concern or medical emergency.
- Teachers, administrators & school staff use the contact information to keep you informed of your child's needs throughout the school year, which includes SCHOOL REACH- an automated communication tool for school announcements & weather related school closures.
- Emergency contacts, Medications, Health conditions, Allergies, and permission for Hydrocortisone cream is requested.
