Allergies
Food Allergies: Food Services must have a completed dietary substitution form signed by your physician documenting the specific food allergy and acceptable substitutes in order to make accommodations within the National School Breakfast and/or Lunch Program. Forms are available from the health office or food services.
EPI-PEN:
In order to be able to administer the Epi pen to your child, we will need:
1. Parent consent
2, A physicians order for this medication and a completed allergy action plan.
Please sign the medication form and fill in the medical provider contact information.
I can assist you with getting your physicians order signed. Please provide me with your physicians phone number and I will fax it for a signature.

When I receive the written permission from you and the Epi pen is in our office, I will write a school emergency plan for your child. This will be shared with school staff and the allergy will be visible to the teachers in the online grade book.
Please note: Ask your medical provider for an extra double pack of the epi pen or twinject. Epinephrine may need to be repeated in 10 to 15 minutes and we prefer to have two injectable epinephrine available at school for emergency use.
Please contact me if you would like to discuss your child’s allergy and emergency care in detail or to schedule a meeting with school staff at the beginning of the school year.
Severe Allergies requiring an epi pen: Please contact the School Nurse to discuss and plan for any special needs that your student has related to allergies.
