? Medications: If your child requires Prescription (RX) or Over the Counter (OTC) medications to be dispensed at school, complete the Medication form and return to your child's school nurse to be approved before the medication can be given.
School Health Services:
A Licensed School Nurse (LSN) is provided by the school district to oversee the Health Services for all of the schools in ISD 578. Our Health Para-professionals assist students with daily acute and chronic health concerns, health documentation, medication administration, immunization records, and communication with parents, teachers, and clinics to optimize student health during the school year -- each day.

- 2 doses of Hepatitis A vaccine are required! This is NEW, (doses given at least 6 months apart)
- 3 doses of Hepatitis B (HBV)
- 5 of DTap*
- 4 of Polio* (IPV)
- 2 of MMR
- 2 of Varicella* (or medical documentation of disease).
- 2 of Hepatitis A
2. Starting 7th grade (new since Fall 2014):
- 1st dose of Meningococcal vaccination is required!
- 3 doses of Hepatitis B (HBV)
- 1 dose of TDaP
- 3 doses of Polio (IPV)
- 2 doses of MMR
- 2 doses of Varicella* (or Medical Documentation of disease)
- (HPV-Human Papillomavirus is recommended after age 11 yrs, but not required)
- Fax, scan, mail or drop off records ASAP to ensure your student is eligible to start school this fall.
